Investigation Summary

Investigation Nr: 170804306
Event: 10/24/1994
Flagger injured when struck by vehicle

At approximately 6:30 a.m. on October 24, 1994, Employee #1, of Roseburg Paving, Roseburg, OR, was flagging traffic in the northbound lane of Highway 99. It was dark out and there was minimal illumination from the two outside lights across the street. A coworker setting traffic cones stated that he had stepped away from the center line and when he looked back he noticed Employee #1 facing him, looking north, and a vehicle about 15 to 20 ft behind him. The coworker yelled at her, but he was too late. She was struck by the vehicle and sustained multiple fractures. Emergency Services (911) was called and Employee #1 was taken to Mercy Medical Center, and later transferred by helicopter to OHSH in Portland and hospitalized.
