Employee's finger crushed when struck by falling object

At approximately 10:00 p.m. on October 21, 1994, Employee #1, a grinder in the cleaning room of Columbia Steel Casting Company, Inc., Portland, OR, was turning over the part he was grinding. The hooks on the jib crane slipped out of the part, allowing it to fall onto the grinding table. Employee #1's left little finger was crushed and he was hospitalized.
