Investigation Summary
At 4:30 a.m. on July 11, 2017, Employee #1 was standing at the baler, feeding cardboard into the machine when an electric pallet jack operated by Employee #2 hit the bin he was removing cardboard from, knocking him to the floor whereupon he hit his head. The employee died July 15, 2017 while at home.
