Investigation Summary

Investigation Nr: 170088934
Event: 05/06/1994
Employee struck by falling logging carriage

Employee #1 was part of a rigging crew that was attempting to shorten a strung-out choker. The signal was given to slack the skidding line so that the 2,000 lb carriage would drift back far enough to hook the choker into the butt hook. The carriage was suspended by the skyline approximately 5 ft from the ground. When the carriage would not drift back, the crew decided to pull it far enough to hook the choker. When the carriage dropped unexpectedly, Employee #1 to lost his balance and fell under it. He suffered a broken shoulder, four broken ribs, and two burst fractures to the lower spine.
