Employee's fingers amputated by table saw blade

At approximately 8:00 a.m. on May 31, 1996, Employee #1 was cutting a piece of Plexiglas with a table saw. The plastic kicked back from the blade and his fingers contacted the unguarded rotating blade. His middle, ring, and little fingers were amputated. He had surgery later that day to repair damage, although there was no reattachment of tissue.
