Employee killed when struck by falling vending machine

Employee #1, a maintenance worker, had completed his assigned task of stocking a closet with supplies for the housekeeping workers at a hotel. He was found dead in an adjacent vending machine room, crushed between a wall and a coin operated vending machine that had fallen forward. The vending machine was serviced by an outside contractor and hotel employees do not work with the machines. There were no witnesses, and it is not known what caused the machine to fall forward.
