Investigation Summary

Investigation Nr: 170570469
Event: 08/24/1994
Employee's leg fractured when caught in auger

Employee #1 was repairing motor bearings on an auger conveyor at the loading and storage area of a vegetable oil processing plant. He brought his leg over the open top of the conveyor. The auger came on and caught his pant leg, pulling the leg into the auger. Employee #1 suffered a crushed leg that was fractured in several places. The conveyor was apparently turned on by another employee because it was not locked out properly. The auger is 8 inches in diameter within a 13 inch wide chute, 16 feet from floor level.
