Investigation Summary
Employee #1 and a coworker were installing sheet metal panels at a roll up door. Both were working from a fully-decked mobile scaffold over 15 feet high when two coworkers attempted to move it to let a forklift pass through the door. No guardrails were around the scaffold work deck. When the scaffold was moved, Employee #1 lost his balance and fell to the concrete floor; he required hospitalization.
