Employee exposed to chemical vapor from spill

At approximately 6:30 a.m. on June 7, 1995, Employee #1 was coming down the stairs from a control room when he noticed a liquid dripping out of a sample port for the reaction. Employee #1 turned off the valve and turned on a water hose that was lying next to the spill. The spill was approximately 18 in. in diameter, and a monitor in the area showed an exposure reading between 200 and 287 ppm at the time of the spill. Employee #1 was exposed for approximately 45 seconds. The following citation was issued for process safety management: T8CCR 1910.119(j)(4)(i).
