Employee killed when he drives scrubber off loading dock

Employee #1 was operating a vehicular-type floor scrubber (Advance model #457610, serial #465190) that weighed 2,885 lb. He was vacuuming water from the loading dock floor when he drove off the dock at the point where trucks back up for loading and unloading. The dock is 50 in. above a concrete floor. The scrubber flipped over and landed on top of Employee #1, crushing his skull and killing him.
