Investigation Summary
Employee #1, a foil mill operator, was threading the foil machine. For an unknown reason, Employee #1, who was performing his usual job duties, stuck his upper body between two rollers that were in the open position. A coworker had seen Employee #1 in his normal upright position just moments before energizing the plunge roller. When the coworker energized the roller, Employee #1 was pinched between the descending plunge roller and the lower shapemaker roller. The coworker immediately hit the emergency stop button, but the descending plunge roller had to run through a complete cycle before it could be raised. Employee #1 was taken to the hospital, where he died 28 days later from medical complications.
