Investigation Summary
Employee #1 was walking across an open yard located at the end of a very large fabrication building when he was struck by a 15-ton Taylor forklift, serial #S-42-11272. Due to the configuration of the hydraulics and mast on this forklift, forward visibility is limited to the area directly in front of the forklift's front left side, where the cab is. The cause of death was multiple forced trauma.
