Employee breaks ankle in 25 foot fall on pipe

Employee #1 was the foreman of a crew that had just used a forklift to lower a scissor lift 25 ft from the upper materials transfer area to the ground. Four 20 ft long uni-strut pipes, weighing approximately 250 lb, were stacked in the transfer area on an ice-covered floor. Employee #1 was approximately 5 feet from the edge (opening) next to the cable marking the area when he saw a pipe start to slide. He attempted to prevent the pipe from falling by straddling it. However, the pipe rose, caught him in the crotch area and then fell off the edge, carrying Employee #1 with it. He landed on the soft muddy ground on his head and back and the pipe landed on his left arm. Employee #1 suffered fractures to his ankle and upper left arm. The employee was not tied off as required; the material transfer area was covered with snow and ice; and material had not been moved from the area in a timely manner. The area was congested with material from another contractor who had let it sit there for an extended period.
