Investigation Nr: 170681621
Event: 12/15/1994
Employee injured when pushed through wall by forklift

Employee #1 and three coworkers were repairing an unstable wall. They had used a forklift to lift a sheet of half inch plywood, and Employee #1 and a coworker then stepped between the forklift and the wall and took hold of the plywood. They instructed the forklift driver to back up. The lift was in the forward gear, but the driver thought it was in reverse. The driver stepped on the throttle and the lift went forward, striking the employees and pushing Employee #1 through the wall. He suffered bruises along the right side of his body and a broken scapula.
