Employee breaks wrist in fall from storage rack

Employee #1 was working in a warehouse with 20 ft tall storage racks. Each rack accommodates three levels. Employees are instructed not to dismount the stacking equipment while it is elevated, and to wear a safety harness while it is raised. Employee #1 was climbing onto a loaded wooden pallet to reach a pallet that was behind it when the wooden pallet he was on broke because of the added weight. Employee #1 fell approximately 15 ft, striking portions of the metal rack and other materials in his descent. He sustained a broken wrist.
