Employee injured when struck on head by falling pipe

Employee #1, a truck driver, was involved in transporting 40-ft sections of 3.5-in.-diameter pipe from one location in an oil field to another location 0.5 mile away. The truck was being loaded by a forklift that was placing four pieces of 450-lb pipe on the truck at a time. The last grouping was being loaded (topped out). Employee #1 was on the end of the flatbed trailer next to the cab of the truck. He was on top of or right next to the load of pipe, which rolled off and struck him on the head. Employee #1 suffered a fractured skull and damage to the nerves that control the muscles in one eye. He was hospitalized for his injuries. Causal factors included the following: the crew was short handed; the forklift tines were 2 in. out of alignment with one another, exceeding the 3-mm tolerance; the pegs on the off side of the flattened truck were only 8 in. high, which was not high enough to contain the top of a 2-ft-high load.
