Investigation Summary
Employee #1 was running a Goss model S-915 Suburban offset printing press in a slow forward direction to make adjustments to the folding portion of the press. He reached up to adjust the press but did not look at the adjustment knob and accidentally put his right middle finger into a set of gears next to that knob. Employee #1 suffered the amputation of his fingertip above the joint.
