Employee's leg broken when caught in roller

A 132 in. rolling mill was shut down for a planned twelve hour downturn. A group lockout was used. While work was being performed by all involved trades, Employee #1, the mechanical repairman, was called to help at an emergency at another location. While he was away, the electrical repairman came to test the mill. All workers in the area were notified that the mill would start up, but no one noticed the absence of Employee #1. The warning horn was not sounded before the rollers were started. Just before the test startup, Employee #1 had returned and gone to the roller area to repair a grease line. When the electrical repairman jogged the rollers, Employee #1 was pulled into the feed rollers and his leg was broken.
