Employee injured when caught in brake roller machine

At 10:30 p.m. on August 16, 1993, Employee #1 was cleaning the in-going rollers of a tandem line brake roller machine. He was hand-cleaning the rollers, wearing gloves, when his arm was caught in the nip point between two of three paint line rollers. Four coworkers either heard Employee #1 yelling or saw him with his arm caught in the machine and came to help. Maintenance and 911 were called. One coworker turned off a fluid control valve to stop the rollers. He then spun the rollers in reverse to release Employee #1. The rewinder was shut off and the material was cut to free Employee #1. A coworker administered first aid to Employee #1, who was hospitalized with lacerations, a wrist fracture, and a shoulder dislocation. It was reported that most of the employees cleaned the rollers in the same way as Employee #1, despite their training.
