Investigation Summary
At 7:00 a.m. on June 26, 2017, an employee was programming and feeding the Chiyoda pipe bender. While the employee was preparing to place a tube 1/2 inch diameter by 4 or 5 feet long to be bent by the Chiyoda machine, he apparently became distracted and placed his hand in the area of operation where the die bends the tube. The employee received an injury on his 5th finger from his right hand, which was later determined to be amputated by the injuries caused.
