Employee killed when crushed by falling extractor

On December 14, 1995, Employee #1 and several coworkers, of the Ford Dearborn Stamping Plant, were setting up a power press on a press line. The press they were working on had a carriage/extractor mounted to it. The extractor had to be moved out of the way. As one of the coworkers operated the extractor's controls, the unit fell to the floor, striking and killing Employee #1. A few days before the accident, an overhead crane with a sling had snagged the extractor and pulled it from the press.
