Investigation Summary

Investigation Nr: 170104053
Event: 06/03/1993
Employee struck and killed by crane superstructure

At approximately 7:25 a.m. after Employee #1 and a coworker, an iron worker, rigged a bundle of corrugated metal decking, it was hoisted to a work area on a roof for installation. The employee and the coworker then went to the side of the 25 ton truck crane to get a 4 lb hammer from a toolbox. The employees were going to use the hammer to take the jib from the boom the next time it was lowered. The hammer was not in the toolbox, so the employees left the area. Employee #1 returned to the toolbox (presumably to look again for the hammer), and was struck by the crane's rotating superstructure. He was pinned between the rear corner of the superstructure and the side of the truck crane. The area where the accident occurred was not barricaded as required.
