Employee's fingers amputated in press

At approximately 8:30 a.m. on May 2, 1994, Employee #1 was investigating an oil leak on a Minster 200-ton, automatic, part-revolution power press. The operator had stopped the press at what he thought was the bottom of its stroke, but had not locked it out. After Employee #1 had put his left hand in the press, the operator realized that the press was not at bottom and went to inch it. When he did, the ram went up instead of down and caught Employee #1's hand between the press frame and the ram. The last two fingers of his hand were amputated and he was hospitalized.
