Investigation Summary
Employee #1 was running a UMC machine, which tightens screws in a starter housing. When a screw would not go into a shoe, the employee unclamped the housing and removed it from the machine arbor to replace the screw and the shoe. She placed the housing back on the arbor, keeping her left hand on the starter housing, and using her right hand to close the air clamp with the air valve. When the clamp snapped shut, it caught her left index finger, amputating it at the first joint.
