Investigation Summary
At 8:45 a.m. on June 9, 2017, an employee was cleaning a screw conveyor to have it ready for the next batch of spices to be blended. The employee removed a plate from the underside of the conveyor and placed a plastic bag at the opening to catch any remaining spices and ran the machine to clear it. While doing so, the employee's right middle finger was caught in the screw conveyor and was partially amputated. The employee was hospitalized.
