Employee injures thumb in power press

At approximately 6:30 p.m. on or about September 13, 1994, Employee #1 was operating the #5 Walsh 65 ton, single stroke, full-revolution, mechanical power press, serial #6397, cutting a 1/8 in. by 1 in. by 1 in. angle iron to 18 in. lengths. His foot operated a guarded foot pedal while he pulled the material to the unguarded point of operation with his right hand and supported the iron against a stop with his left hand. On the sixth piece cut, he tripped the press while his left thumb was caught between the material and the left front corner of the lower die. His left thumb was fractured and severely lacerated. He received emergency medical treatment at Elkhart General Hospital and returned to work that same night.
