Investigation Summary
Employee #1 was operating a 30-ton Bliss full-revolution mechanical power press. She was placing a part in the press when she apparently stepped on the foot switch and activated the machine. The press cycled, breaking the index finger on her left hand. The front barrier guard of the press had been removed.
