Event: 01/20/1994
Employee dies, others hospitalized after exposed to CO

At approximately 1:20 p.m. on January 20, 1994, Employees #2 and #4 discovered Employee #1 unconscious on top of a 97 1/2 inch high water tank in a machine water pump room. Part of Employee #1's job was to check the water level in the tank. Employee #2 lost consciousness and Employee #4 collapsed while trying to rescue Employee #1. Employees #3, #5, #6, and #7 assisted emergency personnel in removing the injured employees. Employee #1 was pronounced dead on arrival at the hospital, and Employees #2 through #7 were treated for carbon monoxide poisoning. The accident occurred because a flare stack exhausting the steel degassing operation was blocked by ice due to the extreme cold, -27 degrees F. The system's back pressure blew a water seal and process gas was forced into a sewer basin, then into the machine water pump room.
