Employee killed when struck on head by beam

Employee #1 had been operating a 10-ton capacity P&H overhead crane, serial #C24930, company #6, for more than two months. He had been using a Telemotive remote, model 8020. A 15 in. by 15 in. 16 ft long steel I-beam (boom) that weighed more than 900 lb was suspended from the hoist by two spreader chains. Short chains were attached to the beam to hook around the load. On the day of the accident, the load comprised pieces of steel bar stock that were 1.6875 in. diameter by 34 ft 2 in. long. The bar stock is placed in a rack area that is approximately 15 ft by 17 ft. The rack has 21 6 ft tall steel uprights that separate the bars in 25 1/2 in. wide racks. Apparently Employee #1 was standing in rack #11 when the beam struck an upright that was approximately 8 ft southeast of him in rack #12. As a result, the beam swung clockwise and struck Employee #1 on the right side of his head, shattering his hard hat and causing a 4 in. laceration. Employee #1 died of a massive skull fracture and brain laceration approximately four hours later.
