EMPLOYEE KILLED WHEN RUN OVER BY TRACTOR TRAILER

EMPLOYEE #1, A TRAINED AND EXPERIENCED TRACTOR OPERATOR, FAILED TO FOLLOW INSTRUCTIONS FOR THE USE OF GEARS AND ROUTING OF A TRACTOR WHILE TOWING A LOADED BIN TRAILER. EMPLOYEE #1 WAS DIRECTED TO USE LOW-RANGE LOW (1ST L) DURING THE DOWNHILL ROUTE; THE TRACTOR MF 255 WAS FOUND IN HIGH-RANGE LOW (1ST H), A DIFFERENCE OF APPROXIMATELY 4 MILES PER HOUR GROUND SPEED. EMPLOYEE #1 ALSO TOOK AN UNAPPROVED ROUTE THAT WAS BOTH DOWNHILL (90 DEGREES FROM THE APPROVED ROUTE) AND SLOPING OFF TO THE RIGHT. ADDITIONALLY, THERE IS EVIDENCE THAT HE WAS EATING A TACO WHILE OPERATING THE TRACTOR. HE ATTEMPTED A 90-DEGREE LEFT TURN, FAILED, CROSSED OVER TO THE ADJACENT ROW, AND IT APPEARS THAT HE MAY HAVE ALTERNATELY ENGAGED THE CLUTCH AND/OR BRAKE MOMENTARILY. THE TRAILER JACKKNIFED, BREAKING OFF THE TRAILER TONGUE, AND THE TRACTOR CONTINUED DOWN SLOPE APPROXIMATELY 30 TO 40 DEGREES FROM THE DOWNHILL ROUTE. DURING THE UNCONTROLLED LEFT TURN, EMPLOYEE #1 EITHER ATTEMPTED TO JUMP CLEAR OR WAS THROWN OVER THE RIGHT SIDE, WHERE HE WAS STRUCK BY A WHEEL AND BROKE SEVERAL RIBS, WHICH LACERATED/PUNCTURED THE LIVER. EMPLOYEE #1 WAS TRANSPORTED TO A LOCAL HOSPITAL WHERE HE WAS PRONOUNCED DEAD.
