Investigation Summary

Investigation Nr: 170872576
Event: 03/12/1999
Employee killed when run over by delivery trailer

Employee #1 was delivering food to a Fresno, CA, distribution center. The facility had a well for a truck to drive into to make the trailer the same level as the loading dock. Employee #1 stopped his vehicle in front of the well so he could open the side doors of the trailer. He got out of the cab and was opening the first door when his truck started to roll forward. Employee #1 ran after it and tried to jump into the cab but the driver's door hit the side of the building and knocked him down. The back wheels of the truck ran over him as the truck rolled through the well. The truck then rolled back into the well and the back tires of the trailer stopped on top of Employee #1. He was pronounced dead at the scene. Employee #1 failed to set the parking brake before pulling the truck forward.
