Investigation Summary

Investigation Nr: 170198782
Event: 11/10/1992
Employee's hand lacerated while replacing valve

Employee #1, an experienced employee of Ban-Terra Transmissions, Inc., installed a replacement pressure relief valve adjacent to a tank. The air compressor was started, bled-off, and restarted to check the system. This cycle was repeated several times. During the operation, Employee #1 noticed the sound of an air leak. While checking for the leak's location with his right hand, the check valve and adjacent plumbing failed. His hand was thrown back against the fins, which severely lacerated the hand. Employee #1 was taken to Kaweah Delta District Hospital, where he was treated, admitted, and released the following day.
