Employee dies after fall from bridge shoring

Employee #1 was a carpenter foreman at the job site for framing a bridge structure. He was climbing a bridge shoring in the northwest corner of the bridge to gain access to a wood plank lying about 12 ft high. Employee #1 was about 10 ft high when he turned his body to reach a 4 in. by 4 in. piece of lumber nailed to the bridge support wall. The nails pulled out of the 4 x 4; Employee #1 lost his balance and fell 10 ft onto a concrete walkway, causing head and other body injuries. He was transported by ambulance to the hospital, where he died 3 days later.
