Employee's leg caught between machine moving parts

Employee #1 was building wooden trusses on a Mark 5 press. The machine was not working properly in that the drive wheel was slipping and would not complete the cycle. Employee #1 and others had to manually push and pull the machine to get it to move. Employee #1 was inside the machine when it moved and caught his lower left leg between the main body and the fixed portion of the machine. Employee #1 received a deep laceration to his lower left leg. The leg later had to be amputated.
