Summary Nr: 97712.015Event: 06/01/2017Employee Fails To Lockout Equipment And Lacerates Hand/Finge
At 4:00 p.m. on June 1, 2017, an employee was performing maintenance on machine. As the employee was cleaning the mechanical roller of the machine, his right h and became caught by the moving parts. The employee sustained severe lacerations on his hand, including injury to his index, middle and ring fingers, which he w as hospitalized and received treatment. The incident investigation revealed that the machine was still plugged in during maintenance and no lockout/tagout proce dures were in place.
