At 4:00 p.m. on June 1, 2017, an employee was performing maintenance on machine. As the employee was cleaning the mechanical roller of the machine, his right hand became caught by the moving parts. The employee sustained severe lacerations on his hand, including injury to his index, middle and ring fingers, which he was hospitalized and received treatment. The incident investigation revealed that the machine was still plugged in during maintenance and no lockout/tagout procedures were in place. | ||||||
