Investigation Summary
On June 29, 1994, Employee #1, a plasterer, was applying stucco to the exterior of a house. He was working from a 10 1/2 ft tubular welded frame scaffold. While Employee #1 was moving from his position on the outrigger to the main body of the scaffold to get a bucket of mud, the scaffold rocked first toward, then away from, the house. Although there were no witnesses to the actual fall, Employee #1 apparently fell backward either over or under the top rail, landing on his neck and head. He was hospitalized and died. The scaffold was not completely cross braced; had inadequate guard rails; was sitting on poor footing; and had a coupler pin missing between the lower and upper sections.
