Employee's arm severely crushed in machine carriage

At approximately 11:30 a.m. on March 9, 1993, Employee #1 was cleaning underneath a Beller machine that forms PVC pipes. Employee #1's right arm was caught in the machine's undercarriage. When the machine cycled, his arm was severely crushed. During surgery a metal plate was placed in his arm.
