Investigation Summary
Shortly after 4:00 p.m. on December 20, 1995, Employee #1, working on the cleanup crew for Taylor Lumber Co., was taking a shortcut between the infeed of a 21 in. debarker and the end of the box-end chain of the log conveyor of the log deck. As he stepped across on a 2 in. by 8 in. plank used to get from the north side to the south side of the upper deck of the debarker, he somehow broke the light beam switch of the press rolls when they were in the raised position. As soon as the light beam was broken, the press rolls descended onto Employee #1, crushing him to death.
