Event: 11/15/1995
Employee killed in fall when panels collapse

At approximately 1:30 p.m. on November 15, 1995, Employee #1 was atop a wood/web truss panel assisting in setting a 37 1/2 ft panel on steel girders. The roof was approximately 30 ft high. As Employee #1 pried with an 18 in. flat pry bar, the panel on which he was standing gave way and became dislodged from one of the girders. It and two other panels collapsed to the concrete floor, causing Employee #1 to fall. He was pronounced dead at the scene.
