Investigation Summary
At about 1:15 p.m. on November 23, 1993, Employee #1, an independent truck driver, was having his curtain-type trailer unloaded by an employee of JOSOCO Industries in White City, Oregon. The forklift driver was attempting to remove the top half of a double-stacked unit of smooth-finished particle board. The unit that was being lifted was the second to last one to come off the trailer. At this time, Employee #1 was on the opposite side, already closing up the curtain of his trailer. Somehow, the particle board unit tipped off the forklift forks and slid off the trailer toward Employee #1, who was cranking the curtain shut. The curtain had not yet been cinched up, so the unit landed on Employee #1, crushing him to death.
