Investigation Summary

Investigation Nr: 170743074
Event: 03/10/1995
Two employees burned in explosion

When Employees #1 and #2 entered a scale house to begin their daily work routine both immediately noticed a rotten meat odor. While Employee #1 started to make coffee, Employee #2 checked on the refrigerator in the scale house. It was closed. Employee #2 then turned on the air conditioner and the computer. An explosion occurred and both employees were engulfed in flames. Employee #1 went to a sliding glass door but found it secured by a lock and wooden dowel. By kicking and striking it, he managed to break the glass, allowing them to escape. A contractor who had seen the explosion applied first aid. Employee #1 sustained second- and third- degree burns and a fractured ankle; Employee #2 sustained second- and third-degree burns to her hands and face. Both were transported by Lifeflight to the burn unit at Emmanuel Hospital, Portland, OR.
