Investigation Summary

Investigation Nr: 170742613
Event: 05/23/1994
Employee injured in fall from elevation

At approximately 5:45 p.m. on or about May 18, 1994, a supplier asked Employee #1 if he could check the float level in the molasses tank. Because he did not have a ladder that could reach the top, Employee #1 asked a coworker if the squeeze would be OK. The coworker agreed and brought the squeeze over to Employee #1 with a unit of hay on it. Employee #1 climbed up onto the hay and the coworker raised the load. When it was all the way up, the coworker put the squeeze into gear to get it closer to the tank. There was a snapping noise and bales started falling. Employee #1 fell approximately 20 ft to the ground, fracturing a bone in his back. Another bale fell and landed on his knee. The molasses supplier transported Employee #1 to the hospital.
