Investigation Summary
At approximately 1:10 p.m. on June 19, 1995, Employee #1, of Cascade Block, was examining an automatic feed system for empty pallets at a cement block manufacturing facility in Medford, OR. While checking the tension on a feed apparatus in the cleaning machine, Employee #1's arm became caught on the carriage works and was shoved into a crossover brace by a hydraulic cylinder. The top of Employee #1's forearm was lacerated to the bone. He was able to extricate himself and was hospitalized.
