Employee killed when struck by sliding log

Employee #1 had been cleaning bark along the bark conveyor belt and log loading deck. He went under the center of the log loading deck and emptied a shovel of bark onto the bark belt. On his way back out, he stopped to watch the log loader operator load logs onto the loading deck. As the loader operator pushed the logs already on the deck forward, a slick log (a log with no bark on it) slid off the loading deck and struck Employee #1 in the chest and head, killing him.
