Employee killed when struck by broken hydraulic line

Employee #1 and five coworkers were tunneling in an approximately 22 to 24 ft deep, 8 to 10 ft wide trench. The day before, a mechanic from the company had replaced a broken 3/4 in. male nipple along with a 3/4 in. male/female quick connect coupling, part #S10S-63500. The parts were installed on the hydraulic jack of a pneumatic clay spade, and the equipment was functioning properly after installation. While in operation the next day, the hydraulic line split laterally from the nipples and broke, striking Employee #1 in the chest and abdomen. He was killed. Trench boxes were being used.
