Investigation Summary
At approximately 6:17 p.m. on July 17, 1999, Employees #1 and #2, along with several other employees, were placing pre-cast concrete beams on a bridge across Dow Chemical shipping canal. Two rainbow beams had already been placed. Employee #1 was standing on and tied to one of the two beams already in place and Employee #2 was working on the north side of the canal. As the third beam was being placed, the crane boom collapsed. Employee #1 fell into the water and drowned. Employee #2 was struck and killed by the falling concrete beam debris.
