Employee crushed and killed when refrigerator overturns

At about 8:20 a.m. on August 20, 1997, Employee #1 and coworkers were loading an approximately 1,200 lb Friedrich refrigerator unit onto a dolly for removal. Two coworkers were using a pry bar to lift one end of the unit and Employee #1 was trying to place the dolly under the raised end when the refrigerator fell over on top of him. Employee #1 sustained crushing injuries and died during Life Flight transport. Causal factors include improper support during lifting of the refrigerator unit and improper training.
