Investigation Summary
Employee #1 was using two forklift trucks to move a tractor trailer into the shop for maintenance. One forklift was placed on the side of the trailer to hold it up while the tractor was disconnected and moved. The second forklift was positioned where the tractor was to push the trailer into the shop. The air line from the tractor to the trailer had been disconnected so the brakes were locked on the trailer. The employee removed the side forklift and pulled the end forklift back so the shop air could be connected to the trailer to release the brakes. When the shop air was connected to the trailer brakes, the brakes released and the trailer rolled forward pinning the employee between the forks of the forklift and the trailer, crushing and killing him.
