Investigation Summary
Employees #1 through #3 and a coworker were using two 3/4-ton come-alongs to slide a crusher slide gate out of a lignite hopper. When the slide gate had been pulled out far enough to clear a sprocket/shaft assembly they were assigned to repair, the workers removed the come-alongs and left the gate unsupported for approximately 3 hours. When they then began work, the slide gate slid out of the hole and struck Employees #1 through #3. They sustained leg, shoulder, and rib fractures. The coworker managed to jump clear and was not injured.
