Employee killed when caught between conveyor and bracing

Employee #1, a production worker in a fried pie manufacturing facility, was told to replace a 4 ft square ceiling panel that had slipped from a suspended ceiling over a carousel conveyor used to cool pies. He entered the conveyor, which was moving at 5 ft per minute, and climbed up the metal braces inside. When he raised his head above the revolving interior conveyor, it became caught against the stationary cross-bracing. His suffered a broken neck and was killed.
